Enroll yourself and your eligible dependents in the Health Plan Add or drop dependents when your family changes, for example, birth, marriage, etc. (Call within 60 days of the event) Apply for short-term disability (accident and sickness) benefits Learn about/enroll in COBRA coverage Learn about/enroll in Retiree Health Plan options Update your address Change payment information (for COBRA and Retiree Plan participants) Note: Address and payment changes must be made in writing and sent to the mailing address to the right. |
